The European Centre for Disease Prevention and Control counted 71 cases by October 21, 62 of them in the Skopje region, and 10 deaths as of October 7. The region around Lake Ohrid reported none, so the Skopje leg of a trip calls for the stronger evening routine, and Ohrid for the usual lakeside one.

Yes, and in 2026 they made the news. North Macedonia had an unprecedented West Nile outbreak this year, almost all of it in the Skopje region, while Ohrid and its lake reported no cases. Wherever you go, wear repellent in the evening, when the mosquitoes that carry West Nile bite, and keep the routine strictest on nights in Skopje.

Skopje and the 2026 West Nile outbreak

The European Centre for Disease Prevention and Control (ECDC) said in 2026 that "North Macedonia is reporting an unprecedented number of human WNV infections." The season started early. The first patient fell ill on May 12, the earliest onset anywhere in Europe that year. ECDC counted 1 case by June 24, 55 by September 3 and 71 by October 21, and 10 people had died as of October 7. For comparison, ECDC counted 2 cases in 2025 and 2 in 2024.

The Skopje region had 62 of the 71 cases. Vardar had 6, and Pelagonia (around Bitola), Polog and the Northeast had 1 each; Pelagonia and Vardar reported human cases for the first time. The country also reported its first West Nile outbreaks in birds this year. Whether 2027 repeats 2026 is unknown, so check CDC and ECDC for the latest picture before a late-summer trip.

West Nile spreads through Culex mosquitoes that bite in the evening and at night. In Skopje, put repellent on before dusk for the Old Bazaar's terraces and again after dark, wear long sleeves for dinner outside, and sleep in an air-conditioned room or with the windows closed. There is no vaccine, and travelers 65 and older or with a weakened immune system should take that routine most seriously in late summer. The day-biting tiger mosquito is spreading too. ECDC's April 2026 map added three areas of North Macedonia where it is established, so carry the spray by day in Skopje and on a Matka Canyon outing.

Ohrid and the lake

Ohrid is the easier half of the trip. ECDC listed no West Nile cases in its region in 2024, 2025 or 2026, so an Ohrid-only trip sits outside the outbreak area, while a night or two in Skopje on the way in or out puts you inside it for those evenings. The lake still brings some mosquitoes on summer evenings, and a lakeside room with the balcony door left open tends to let them in. Put repellent on for the promenade and lakeside terraces at dusk, and keep balcony doors closed or screened after dark.

Sand flies are the other night biter. CDC's North Macedonia page lists leishmaniasis, which they spread, and the visceral form is reported at a low rate, in the same range as Greece and Spain. Evening repellent covers them along with the mosquitoes. Our guide to how long repellent lasts helps you time the top-up.

The North Macedonia kit

The same spray covers both halves of the trip. Skopje in late summer is where to use it most.
  • Picaridin 20% travel sprays: odorless for evenings at the Old Bazaar and on Ohrid's promenade, and small enough to carry by day in Skopje. One 3.4 oz bottle covers about a week of daily use for one person.
  • A plug-in vaporizer for lakeside rooms and warm nights with the window open.
  • A light long-sleeved layer for dinners outside in Skopje.

What travelers report

Paraphrased from a TripAdvisor forum thread and linked so you can read it in full. It is about three years old and predates the 2026 outbreak.

  • Ohrid, Bitola, Mavrovo and Skopje · September (thread); summer

Asked about September mosquitoes, one reply said they had no major problems and that the government sprays against larvae every May. Another had no trouble in Ohrid, Bitola or Mavrovo that summer but was attacked even in daytime in the suburbs of Skopje, and said the tiger mosquito had been active in Macedonia for a few years. The thread is about three years old. TripAdvisor ↗

One more rule outlasts the trip. Any fever during or after your trip means a doctor visit, and you tell them you were in North Macedonia and whether you spent time in Skopje.

Traveler questions

What does the 2026 West Nile outbreak mean for a Skopje visit?

A stronger evening routine. The European Centre for Disease Prevention and Control counted 71 cases in North Macedonia by October 21, 2026, 62 of them in the Skopje region, and 10 deaths as of October 7. The mosquitoes that carry West Nile bite in the evening and at night, so repellent before dusk, long sleeves for dinners outside and an air-conditioned room or closed windows at night are the protection. There is no vaccine, and travelers 65 and older or with a weakened immune system should be most careful in late summer.

Were there West Nile cases in Ohrid?

No. ECDC listed no cases in the region around Ohrid in 2024, 2025 or 2026. Lake evenings still bring some mosquitoes, so wear repellent at dusk on the promenade and keep the balcony door closed after dark. If you fly into Skopje, the evenings you spend there are the ones inside the outbreak area.

Will West Nile be back in 2027?

No one knows yet. In 2026 the first case fell ill on May 12, only 1 case had been reported by June 24, and most were reported between late June and early September. Across Europe, cases peaked at the end of August. Before a summer trip, check CDC and ECDC for the latest picture.

Do I need malaria pills or vaccines for North Macedonia?

No malaria medicine. WHO certified North Macedonia malaria-free in 1973, and CDC lists no malaria medicine for it. The yellow fever vaccine is neither recommended nor required, and there is no vaccine for West Nile.
