Placencia's sand flies decide the trip, and the wind decides the sand flies.
- No CDC travel notice for mosquito-borne disease
The peninsula's biting midges, called sand flies locally, give it Belize's strongest bite reputation, worst at dawn, at dusk and on still evenings by the lagoon. Dengue is endemic in Stann Creek, and the CDC lists no malaria medicine for Belize.
Yes, and on the Placencia peninsula the bites you'll notice most come from sand flies, the tiny biting midges called no-see-ums in the US, at dawn and dusk. Mosquitoes matter too, because dengue is endemic and its mosquito bites by day. The CDC lists no malaria medicine for Belize.
Sand flies on the beach side and the lagoon side
Placencia has the strongest sand-fly reputation in Belize. The biters are midges, weak fliers that go for legs, ankles and feet and come out mostly at dawn and dusk. Forum posters tend to say a breeze helps without reliably driving them off, that they get through most screens, and that most beachfront places rake the sand daily to keep them down. Several posters report few or no bites at all.
The peninsula has two sides. The east side, from Maya Beach and Seine Bight down to Placencia Village, faces the Caribbean, the trade wind and the daily raking. The west side faces the sheltered Placencia Lagoon, with mangrove edges, docks and still air, and one lagoon-side visitor said the flies came out at dusk around the pool. A calm evening on the lagoon side is likely the worst moment of the trip.
Put an EPA-registered repellent on legs, ankles and feet before dawn and dusk and after every swim, and pick a breezy beach-side room or a well-screened, air-conditioned one. Coconut oil, baby oil and Skin So Soft are local favorites, but they aren't EPA-registered repellents, so keep a registered one as the base layer. Our Barbados sand fly guide explains how midges behave, and the Ambergris Caye briefing covers the northern cayes.
Dengue, and what the CDC says about malaria
Dengue is endemic in Belize, and the CDC rates the risk frequent or continuous. Stann Creek, Placencia's district, has carried a large share of recent seasons, with about a quarter of Belize's 2024 cases, local news reported. In October 2026 the Pan American Health Organization reported dengue rising again across Central America. Repellent over sunscreen in the morning and late afternoon covers the dengue mosquito.
The CDC's Belize page names no malaria areas, and its vaccines and medicines list has no malaria entry. Its general travel tips still say, "Malaria is a risk in some parts of Belize." The World Health Organization certified Belize malaria-free in 2023. No yellow fever vaccine is recommended or required. Adding Tikal? The CDC recommends malaria medicine for Guatemala's Petén department, so see a travel clinic 4–6 weeks before departure.
Jungle tours and the reef
Reviewers of the Monkey River trip, a boat ride down the coast and up the river into Toledo district, tend to call the jungle walk the buggy part of the day, with heavy mosquitoes and doctor flies. Cockscomb Basin's forest trails have a similar reputation. Wear long sleeves, trousers and closed shoes, ideally permethrin-treated, carry repellent and check for ticks afterward. The forest also holds a different sand fly, the phlebotomine kind that spreads leishmaniasis and bites mainly from dusk to dawn, so limit time outdoors in the forest at night, as the CDC advises. Snorkel trips to the small, breezy Laughing Bird Caye and Silk Cayes are the easy part.
Where you're going
Scores are for a December–April trip.
The lagoon side
The sheltered west shore, with mangrove edges, docks and still air. Sand flies come out at dusk, so cover up or move away from the docks at sunset.
Monkey River tour
A boat down the coast and up the river to a jungle walk, the buggy part of the day. Wear long sleeves, trousers and closed shoes, and check for ticks afterward.
Cockscomb Basin
Rainforest trails and waterfall pools inland. Long sleeves and trousers help by day, and forest after dark brings the sand flies that spread leishmaniasis.
Laughing Bird Caye & the Silk Cayes
Small, open reef cayes reached by snorkel boat. The breeze tends to keep midges down; still afternoons on the sand are the exception.
The Belize briefing covers the whole country, and the San Ignacio & Cayo briefing covers the jungle lodges and caves inland.
The Placencia kit
- Picaridin 20% travel sprays: odorless in the heat, layers over sunscreen and deters midges as well as mosquitoes. One 3.4 oz bottle covers about a week of daily use for one person.
- Permethrin 0.5% for tour clothes: treat a long-sleeved shirt, trousers and socks at home 48 hours before you fly. It repels ticks as well as insects, and it goes on clothing, never on skin.
What travelers report
Paraphrased from a TripAdvisor thread and a tour review and linked so you can read them in full. Neither gives a year, so read them as the general picture.
- Placencia · Winter
A winter visitor bitten repeatedly on the first day asked for help; replies said the biters are sand flies, that DEET works for some people but not all, and suggested antihistamine and hydrocortisone cream. One reply said a visitor who bought a Cutter spray had no more bites, and another poster found sand flies only in Placencia, not in Corozal or Cayo. TripAdvisor ↗
- Monkey River Eco Tours (Monkey River tour from Placencia)
A reviewer was bitten by a tick on the tour and advised other visitors to cover up and check themselves afterwards. TripAdvisor ↗
One rule outlasts the trip. Any fever during or after the trip means a doctor visit, and you tell the doctor you were in Belize. The same goes for a bite sore that won't heal.
Traveler questions
Do I need malaria pills for Placencia?
The CDC's Belize page names no malaria areas, and its vaccines and medicines list has no malaria entry. Its general travel tips still say "Malaria is a risk in some parts of Belize." The World Health Organization certified Belize malaria-free in 2023. If you're adding Tikal, where the CDC recommends malaria medicine for Guatemala's Petén department, see a travel clinic 4–6 weeks before departure.
What works against Placencia's sand flies?
An EPA-registered repellent such as picaridin 20% on legs, ankles and feet before dawn and dusk and after every swim, plus a breezy or well-screened, air-conditioned room. Coconut oil, baby oil and Skin So Soft are local favorites, but they aren't EPA-registered repellents, so keep a registered one as the base layer.
Is the beach side or the lagoon side better for bites?
The beach side usually has the edge. It faces the Caribbean trade wind, and most beachfront places rake the sand daily. The lagoon side is sheltered, with mangroves, docks and still air, and a calm evening there is likely the worst moment of the trip.
Are Placencia's sand flies the ones that spread leishmaniasis?
No. The beach sand flies are biting midges, the no-see-ums of the US. Leishmaniasis, which the CDC lists for Belize, spreads through phlebotomine sand flies that live in forest and bite mainly from dusk to dawn. That matters for Cockscomb and Monkey River, where the CDC advises limiting time outdoors in the forest at night.
