Inland from the cayes, San Ignacio's jungle lodges, caves and rivers bring forest mosquitoes, ticks, the sand flies behind a skin infection and the odd botfly. Belize was certified malaria-free in 2023, dengue is the disease to plan around, and permethrin-treated clothes do much of the work.

Are there mosquitoes in San Ignacio? Some, though dry-season visitors tend to report modest numbers. Cayo is inland forest and river country, where forest mosquitoes, ticks, the sand flies behind a skin infection and the occasional botfly take over from the beach sand flies of the cayes. Treated clothes matter as much as repellent here.

Disease risk ModerateDengue is endemic, and Cayo had 225 of Belize's 865 cases by July 2024; the forest adds leishmaniasis, and there are no 2026 Belize figures.

Dengue is the disease to plan around

Dengue is endemic in Belize, and the CDC places the country in its group with frequent or continuous risk. By July 2024, 225 of Belize's 865 cases were in Cayo, the Ministry of Health's vector control operations chief told Amandala. In late November 2025 the San Pedro Sun reported more than 600 confirmed cases nationwide, most in Orange Walk and Corozal. There are no 2026 figures for Belize or Cayo, and in October 2026 the Pan American Health Organization reported dengue rising again across Central America.

The dengue mosquito bites by day, so repellent goes on over sunscreen in the morning, again before dusk and after every river wade or swim. Chikungunya and Zika, spread by the same mosquito, have been reported in recent years, less than in Guatemala.

Malaria-free, with one CDC line to know

The World Health Organization certified Belize malaria-free in 2023, and the CDC's Belize page lists no malaria medicine in its vaccines and medicines section. Its general travel tips still say, "Malaria is a risk in some parts of Belize." The two don't line up, so take your itinerary to a travel clinic 4–6 weeks before departure. That visit matters more if you're adding Tikal, since the CDC recommends malaria medicine for Guatemala's Petén department, which includes Tikal and the border town of Melchor de Mencos.

Forest biters: sand flies, ticks and botflies

Cayo's phlebotomine sand flies bite mainly from dusk to dawn and can spread cutaneous leishmaniasis, a skin infection the CDC considers endemic in Belize. The risk is higher for ecotourists, people outdoors for long hours and travelers in rustic lodging, and British and Dutch troops have caught it on jungle training here. The CDC advises limiting time outdoors at night in the forest and sleeping in an air-conditioned or well-screened room.

Ticks are the daytime forest risk. In a 2014–15 study, some of the ticks collected in 23 villages in northern and western Belize carried spotted fever group Rickettsia, which the researchers said suggests a risk of tick-borne disease.

Botflies are uncommon on people but documented in travelers. A review of traveler cases from 1999 to 2015 found the highest rates of botfly infestation in travelers to Belize, Bolivia and Brazil, and a 2024 report described a US traveler who needed surgery to remove a botfly larva from his knee after a Belize trip. The botfly tends to use mosquitoes and other biting insects to carry its eggs, so protection against mosquitoes helps here too.

Treat a long-sleeved shirt, trousers and socks with permethrin at home and wear them on the ATM trail, Mountain Pine Ridge hikes and lodge walks, trousers tucked into socks, and check for ticks each evening. After the trip, see a doctor about a boil-like bump that keeps growing or a sore that won't heal.

Where you're going

Scores are for a December–April trip.

jungle

Actun Tunichil Muknal (ATM) cave

Disease risk ModerateBite nuisance Noticeable

A forest trail, river crossings and a wade into the cave, so the trail is the buggy part and the water strips repellent. Wear treated clothes on the hike and reapply once you're dry.

countryside

Xunantunich & the Mopan River ferry

Disease risk ModerateBite nuisance Occasional

An open hilltop ruin reached by a hand-cranked river ferry and visited by day, when the dengue mosquito bites. The riverbank and the forest edge are the likelier bite spots.

jungle

Macal River jungle lodges

Disease risk ModerateBite nuisance Noticeable

Rustic or open-air lodging raises the leishmaniasis risk from sand flies that bite from dusk to dawn. A screened or air-conditioned room helps, and expect plenty of other insects in a jungle cabin.

highland

Mountain Pine Ridge

Disease risk LowBite nuisance Occasional

Higher, cooler pine forest with waterfalls and pools, likely with fewer mosquitoes than the river valleys. Grassy areas can hold ticks, and every swim means fresh repellent.

The Belize briefing covers Placencia and the cruise port, and our Ambergris Caye & Caye Caulker briefing covers the beach sand flies. Adding Tikal? The Tikal & Flores briefing has Petén's malaria advice.

The Cayo kit

Treated clothes for the trails and the lodge, and a spray for the skin they leave out.
  • Permethrin 0.5% for jungle clothes: treated fabric repels and kills insects and ticks on contact through several washes. Treat at home 48 hours before you fly. It goes on clothing, never on skin.
  • Picaridin 20% travel sprays: odorless, layers over sunscreen and is quick to reapply after the cave or a waterfall swim. One 3.4 oz bottle covers about a week of daily use for one person.

What travelers report

Paraphrased from TripAdvisor forum threads and a lodge review and linked so you can read them in full. None is dated, so read them for the pattern rather than for this season.

  • The Lodge at Chaa Creek, San Ignacio (Cayo)

A guest at this jungle lodge reported no bites and said mosquitoes were not too bad, though plenty of other insects turned up in the cabin. TripAdvisor ↗

  • San Ignacio (and Ambergris Caye) · late March to early April

Replies to a traveler asking about mosquitoes in late March and early April said the spring months are usually dry and breezy and mosquitoes struggle in wind, so San Ignacio and Ambergris Caye should be fine; long sleeves and DEET matter mainly for jungle stays in the rainy season. TripAdvisor ↗

  • Belize jungle areas

Asked about botflies, a poster said they are found only in the jungles and are very, very uncommon on humans. TripAdvisor ↗

One rule outlasts the trip. Any fever during or after your trip means a doctor visit, and you tell them you were in Belize, and in Guatemala if you went on to Tikal.

Traveler questions

Do I need malaria pills for San Ignacio?

The World Health Organization certified Belize malaria-free in 2023, and the CDC's Belize page lists no malaria medicine in its vaccines and medicines section. Its general travel tips still say malaria is a risk in some parts of Belize, so take your itinerary to a travel clinic 4–6 weeks before departure. That visit matters more if you're adding Tikal, because the CDC recommends malaria medicine for Guatemala's Petén department.

How common are botflies in Belize?

Uncommon on people, but documented in travelers. A review of traveler cases from 1999 to 2015 found the highest rates of botfly infestation in travelers to Belize, Bolivia and Brazil. Repellent and long sleeves against mosquitoes help, since the botfly tends to use other biting insects to carry its eggs. See a doctor about any boil-like bump that keeps growing after the trip.

What should I wear for the ATM cave tour?

The tour combines a forest hike, river crossings and a wade and swim into the cave. Permethrin-treated long trousers and a long-sleeved shirt help on the trail, but water strips skin repellent, so reapply once you're out and dry.

Are ticks a problem in Cayo?

They are a forest risk. A study of ticks collected in villages in northern and western Belize found spotted fever group Rickettsia in some samples, which the researchers said suggests a risk of tick-borne disease. Tuck trousers into socks on trails and in grassy areas such as Mountain Pine Ridge, and check yourself each evening.
