Are there mosquitoes in Cozumel? A few, mostly in town.
- CDC Level 1 Rocky Mountain spotted fever (ticks), northern border states only
Cozumel had Quintana Roo's highest dengue rate in 2025, but the island has been quiet in 2026 even as the state's count climbed. December to April is the dry low season, and a cruise day or a dive week calls for daytime repellent and not much more.
A few, and mostly in town. The day-biting Aedes mosquito that spreads dengue breeds around homes in San Miguel, while a dry-season port day at the piers or a week spent on dive boats tends to see few bites. Cozumel had Quintana Roo's highest dengue rate in 2025, but the island has been quiet in 2026.
Dengue: a busy 2025, a quiet 2026
Cozumel belongs to Quintana Roo, the state that holds Cancún and Tulum. In 2025 the island had 65 confirmed dengue cases, the state's highest rate at about 57 per 100,000 people, according to Mexico's federal epidemiology directorate. This year has been different. Cozumel had 3 confirmed cases from January to late April, and in the mid-October bulletin it wasn't among the four municipalities the directorate names. Solidaridad (Playa del Carmen) led with 134, followed by Othón P. Blanco (Chetumal) with 108.
The state as a whole is having a big year. Mexico's health ministry counted 468 confirmed cases in Quintana Roo by October 19, 2026, more than double the 204 at the same point in 2025 and above the 287 for all of 2025. On the island, the newspaper Por Esto reported in August that mosquito monitoring found the most activity in the San Miguel 1 and San Miguel 2 neighborhoods of town.
Chikungunya spreads through the same mosquito. Quintana Roo had 6 of Mexico's 15 confirmed cases for 2026 in the early-October bulletin, which doesn't say which municipalities they were in. Mexico reported no Zika, West Nile or malaria in the state through early October. The CDC's malaria areas for Mexico are "Campeche, Chiapas, and the southern part of Chihuahua state," so it recommends no malaria medicine for Cozumel.
Cruise day or dive week
A port call falls in daylight, which is when the dengue mosquito bites. Put repellent on over sunscreen before you leave the ship and again after swimming. Divers spend most of the day on boats, where bites are rare. If your dive boat or reef park asks for reef-safe products, put repellent on after the dive rather than before you get in the water.
A stay of a few nights adds town and dusk. Keep balcony doors shut and the air-conditioning on, especially in December or after rain. The undeveloped east side is usually breezy, which keeps biting midges down, while the lagoons and mangroves at Punta Sur and on the north end are likelier to bite on a calm evening. Spray ankles and calves for those. Our Barbados sand fly guide explains how midges behave.
Where you're going
Scores are for a December–April trip.
San Miguel: town & malecón
Where the island's dengue has clustered, with the San Miguel 1 and 2 neighborhoods showing the most mosquito activity in 2026 monitoring. Rated for a stay that may start in December; from January to April it drops a step.
Cruise piers & west-coast beach clubs
A daylight port call in the dry season means limited exposure. Put repellent on before you leave the ship and again after swimming.
The east side: Punta Morena, San Martín & Chen Río
The undeveloped windward coast, where the usual breeze keeps biting midges down. A calm evening may bite more, so spray your ankles if you stay for sunset.
Punta Sur & the southern dive resorts
The park's lagoon and mangroves are likelier spots for mosquitoes and midges at dusk than the reef-side resorts. Days on a dive boat see few bites.
Our Cancún & Riviera Maya briefing covers the mainland coast across the strait, and the Mexico briefing compares every region.
The Cozumel kit
- Picaridin 20% travel sprays: odorless in the heat, layers over sunscreen and fits a beach bag on a port day. One 3.4 oz bottle covers about a week of daily use for one person.
- Oil of lemon eucalyptus 30%: the plant-based option among EPA-registered repellents. Not for children under 3.
What travelers report
Paraphrased from a cruise forum thread and linked so you can read it in full. It's undated and older, and the dry-season picture on the island is usually lighter.
- Cozumel and Calica (Playa del Carmen cruise pier)
In an older cruise thread, one traveler said the mosquitoes were vicious on stops at Calica and Cozumel, while another said the bug spray they packed seemed to save them and that the pests were not too bad right on the beach. Cruise Critic ↗
One rule outlasts the trip. A fever during the trip or within two weeks of returning means a doctor visit, and you tell the doctor you were in Quintana Roo. If you were bitten and want to know what to watch for, read bitten in a dengue zone.
Traveler questions
Are there mosquitoes in Cozumel in January?
Fewer than at any other time of year. January falls in the dry season, the low point for dengue, and Quintana Roo had only 25 confirmed cases statewide by early February 2026. The dengue mosquito still bites by day, especially after an unseasonal rain, so keep the morning repellent habit.
Do I need repellent for a Cozumel cruise stop?
Bring it. A dry-season port day likely means limited exposure, but the dengue mosquito bites in daylight, which is when you're ashore. Put repellent on over sunscreen before you leave the ship and top it up after swimming.
Do I need malaria pills for Cozumel?
No. The CDC's malaria areas for Mexico are Campeche, Chiapas and southern Chihuahua, and it recommends no malaria medicine for Quintana Roo. Mexico's bulletin reported no malaria in the state through early October 2026. The CDC also doesn't recommend the yellow fever vaccine for Mexico, and it suggests the chikungunya vaccine only for stays of about six months or more.
Is Cozumel quieter than the mainland coast for dengue?
In 2026, yes. Cozumel isn't among the four Quintana Roo municipalities the federal bulletin names this year, while Solidaridad, which holds Playa del Carmen, had 134 confirmed cases by mid-October. In 2025 it was the other way around, and Cozumel had the state's highest rate. Same state, same day-biting mosquito, so the same routine works on both sides of the strait.
