Caye Caulker's sand flies follow the wind, and the lagoon side gets less of it.

Belize was certified malaria-free in 2023, and the cayes' district has carried a small share of recent dengue seasons. The bites you'll notice are sand flies, tiny midges that come out before sundown and on still days, and the sheltered lagoon-side docks have a reputation as the worst spot.

Yes, though the bite you'll notice on Caye Caulker is usually a sand fly, the tiny biting midge Americans call a no-see-um. Belize was certified malaria-free in 2023, and recent dengue seasons have centered on the mainland. Sand flies come out before sundown and on still days, and the steadier the wind, the fewer you'll meet.

Disease risk LowMalaria-free since 2023 with no CDC malaria medicine; dengue is endemic, but the cayes' district had about a tenth of Belize's 2024 cases, and there are no 2026 figures.

Dec–Apr tripsThe drier half of the year, so dengue pressure is lower, and February to April is the lower-risk stretch. Sand flies follow the wind, and a still winter evening on the lagoon side can bite as hard as any summer night.

East side, lagoon side and the Split

Caye Caulker's sand flies are weak fliers that bite ankles and lower legs, and they have a reputation for coming out just before sundown and on any still day. The east side faces the reef and gets the steadier trade wind. The west side faces the sheltered lagoon, with mangrove edges, docks and less wind, and it's where the island gathers for sunset, so a still evening on a west-side dock is likely the itchiest moment of the trip. The north island across the Split has a similar reputation.

The Split, the channel at the north end of the village where most visitors swim, is breezy when the trade wind blows, but you sit on sea wall, docks and a bar deck there rather than on raked sand. Reapply after every swim, and spray feet, ankles and calves before you go down for sunset. Reef and manatee snorkel trips run over open water and tend to be breezy and largely bite-free. Our sand fly guide explains the insect.

The local tip is coconut oil or baby oil, sold in small bottles in island groceries. Neither is an EPA-registered repellent, so add it over a registered product rather than using it instead.

How it differs from Ambergris

Caye Caulker shares Ambergris Caye's disease picture, and the setting is what differs. On the much larger Ambergris, San Pedro's resort beachfront is raked daily. Caye Caulker has almost no natural beach, so dawn and dusk happen on docks, sea walls and the Split, and every guesthouse is a few minutes' walk from the lagoon side. On a still day that can make Caye Caulker feel worse. Our Ambergris Caye briefing covers San Pedro, and the Placencia briefing covers the mainland peninsula with the strongest sand-fly reputation in Belize.

Dengue is endemic, and quieter on the cayes

Dengue is endemic in Belize, and the CDC places the country in its frequent-or-continuous risk group. Caye Caulker belongs to Belize District, which had roughly a tenth of the cases Belize counted by mid-December 2024, local news reported, while Stann Creek, Toledo and the northern districts led. There are no 2026 figures for Belize, and in October 2026 the Pan American Health Organization reported dengue rising again across Central America.

The dengue mosquito bites by day in the village, with peaks after sunrise and in the late afternoon, so repellent goes on over sunscreen in the morning and again before sunset. Our dengue & Zika routine has the full schedule. Chikungunya and Zika have been reported in Belize, less than in Guatemala, and the same habit covers them.

The CDC's Belize page lists no malaria medicine, the World Health Organization certified Belize malaria-free in 2023, and the CDC does not recommend the yellow fever vaccine. A Tikal side trip changes the malaria question, because the CDC recommends medicine for Guatemala's Petén department, so see a travel clinic 4–6 weeks before departure if it's on the plan.

Scores are for a December–April trip.

city

Front Street & the east side

Disease risk LowBite nuisance Occasional

The village's main sand street and the reef-facing shore, where the steadier trade wind tends to keep sand flies down. The dengue mosquito bites by day in the village, so the morning repellent habit applies.

The Split

Disease risk LowBite nuisance Noticeable

The swimming spot at the north end of the village, breezy when the trade wind blows, with sea wall and docks rather than sand. Reapply after every swim and spray feet and ankles before sunset.

West-side sunset docks & lagoon bars

Disease risk LowBite nuisance Noticeable

The island's sunset spot, on sheltered water with mangrove edges. A still evening here has the worst sand-fly reputation on the island, so cover or spray your legs before sundown.

island

North Caye Caulker

Disease risk LowBite nuisance Annoying

The quieter island across the Split, with mangroves and a small forest reserve at the tip. It has a reputation for sand flies, so carry repellent for the crossing.

The Belize briefing covers Cayo, the cruise port and the rest of the country.

The Caye Caulker kit

The spray handles the daytime mosquitoes and the sundown sand flies alike.
  • Picaridin 20% travel sprays: odorless in the heat, layers over sunscreen and deters sand flies as well as mosquitoes. One 3.4 oz bottle covers about a week of daily use for one person.
  • A full-size backup in the checked bag. Reapplying after every swim at the Split goes through a bottle quickly.
  • Light long trousers for evening drinks on the lagoon side.

What travelers report

Paraphrased from a TripAdvisor thread and a guest review, linked so you can read them in full. Neither is dated, and on the cayes the wind on the day matters more than the year.

  • Caye Caulker

In a thread on oil-based protection, a reply said coconut oil or baby oil both work against sand flies but coconut oil is the more common local recommendation, and that small bottles are sold in the island's grocery stores. TripAdvisor ↗

  • Bay Towers, Caye Caulker

A guest complained of so many sand flea bites that it looked like measles; the management reply said the property is treated regularly but that weather, including rainy periods, affects their numbers. TripAdvisor ↗

One rule outlasts the trip. Any fever during or after your trip means a doctor visit, and you tell them you were in Belize, and in Guatemala if you went on to Tikal.

Traveler questions

Which side of Caye Caulker has fewer sand flies?

The east, reef-facing side usually has the edge, because it gets the steadier trade wind. The west, lagoon side is sheltered, with mangrove edges and docks, and a still sunset there tends to be the island's worst moment for bites. Wherever you stay, spray feet, ankles and calves before sundown and pick a room with good screens or air-conditioning.

Is Caye Caulker worse than Ambergris Caye for sand flies?

It can feel that way on still days. San Pedro's resort beachfront on Ambergris is raked daily, while Caye Caulker has little natural beach, so evenings happen on docks, sea walls and the Split, close to the lagoon side. In a steady breeze both islands are easy.

Is dengue a risk on Caye Caulker this winter?

Dengue is endemic in Belize, but recent seasons centered on mainland districts, and Belize District, which includes the cayes, had about a tenth of the country's 2024 cases. There are no 2026 Belize figures, and in October 2026 PAHO reported dengue rising across Central America. The mosquito bites by day in the village, so repellent goes on over sunscreen every morning.

Do I need malaria pills for Caye Caulker?

The CDC's Belize page lists no malaria medicine, and the World Health Organization certified Belize malaria-free in 2023. The CDC's general tips still say malaria is a risk in some parts of Belize, so if the trip adds Tikal in Guatemala's Petén department, where the CDC recommends malaria medicine, see a travel clinic 4–6 weeks before departure.
