Kandy had a hard dengue year. The Cultural Triangle adds sand flies at dusk.
- CDC Level 1 Global dengue (Sri Lanka listed)
Press reports of national dengue unit data put Kandy among Sri Lanka's hardest-hit districts in 2026. In the lowlands around Sigiriya, Dambulla and Polonnaruwa, sand flies that spread leishmaniasis bite from dusk to dawn. There is no malaria.
Yes. Kandy was among Sri Lanka's hardest-hit districts in the 2026 dengue epidemic, so the day-biting mosquitoes matter even in the cooler hills, and the lowland Cultural Triangle around Sigiriya, Dambulla and Polonnaruwa adds sand flies at dusk. There is no malaria. Our Sri Lanka briefing covers Colombo, Ella and the rest of the island.
Kandy's dengue year
Sri Lanka's 2026 epidemic passed 100,000 cases by September 27, more than half of them in the Western Province around Colombo, according to National Dengue Control Unit figures reported by the European Centre for Disease Prevention and Control. Kandy district, in the Central Province, was among the hardest-hit districts, according to press reports of the unit's data. The unit flagged it in January, in areas hit by the floods of Cyclone Ditwah, and in June health officials named Galagedara, Gampola and Udapalatha among the district's high-risk areas, Sri Lankan media reported. Nationally, cases peaked in July and were falling by September.
Dengue spreads through Aedes mosquitoes, which bite by day with peaks after sunrise and in the late afternoon. Kandy sits at roughly 1,600 feet (500 m), and the hills tend to be lighter on bites than the lowlands, but the mosquitoes are in its gardens, hillside guesthouses and the paths around the lake. Put repellent on over sunscreen after breakfast, again after a sweaty climb and before the late afternoon, and sleep in an air-conditioned room or under a net without holes. Our dengue & Zika routine has the full schedule.
Sand flies in the Cultural Triangle
Sigiriya and Dambulla are in Matale district, part of the Central Province with Kandy. Polonnaruwa and Anuradhapura, farther north, make up the North Central Province. The Triangle is lowland dry zone, and its irrigation tanks and paddy fields tend to bring more mosquitoes than Kandy does.
It is also a leishmaniasis hotspot. The skin infection, which the CDC lists for Sri Lanka, is spread by phlebotomine sand flies that bite from dusk to dawn. A 2025 study in PLoS Neglected Tropical Diseases put Polonnaruwa's average yearly rate in 2009–2023 at 52.21 cases per 100,000 people, second only to Hambantota in the south, with Anuradhapura at 40.30 and Matale at 25.48. Anuradhapura's and Matale's rates rose steadily after 2015, and cases peak around June to August. Kandy district isn't among the leading districts. Evening jeep safaris to Minneriya or Kaudulla, sunset on Pidurangala and outdoor lodge dinners are the exposure, so wear long sleeves and trousers from dusk, ideally treated with permethrin.
No malaria, one vaccine question
WHO certified Sri Lanka malaria-free in 2016, and the CDC recommends no malaria medicine for the country. Japanese encephalitis is present, and the CDC's advice depends on the trip. A Kandy stay fits its "short-term travel to urban areas," while homestays among the paddies come closer to its "visiting rural areas," as the FAQ below explains. Pregnant travelers should talk to a provider, since the CDC lists Zika for Sri Lanka. Yellow fever vaccine isn't needed when you fly from the US.
When to go
Roughly February to April tends to be the easiest stretch, after the northeast monsoon's rains in the dry zone and before the mid-year peaks. June to August is the hardest, with the 2026 dengue peak and the leishmaniasis season. Kandy's Esala Perahera festival usually falls in July or August, inside that window. No month is dengue-free.
Where you're going
Kandy Lake & the Temple of the Tooth
The center of one of 2026's hardest-hit dengue districts. Day-biting mosquitoes live in gardens, guesthouses and along the lakeside paths, though the hill climate tends to keep bites lighter than in the lowlands.
Sigiriya & Pidurangala
Matale district, Central Province, where the leishmaniasis rate has risen since 2015. The rock climb is a daytime visit in dengue-mosquito hours, and sunset on Pidurangala runs into sand fly hours, so cover up for the walk down.
Dambulla Cave Temple
Also in Matale district, on the road between Kandy and Sigiriya. Repellent goes on before the climb to the caves. Triangle hotels vary on nets and screens, so check your room.
Polonnaruwa
North Central Province, with Sri Lanka's second-highest leishmaniasis rate in 2009–2023. Long sleeves and trousers for any time at the ruins or the lodges after dusk.
Heading to the beaches afterward? Our Galle and south coast briefing covers Galle Fort, Mirissa and Yala.
The Kandy and Triangle kit
- Picaridin 20% travel sprays: odorless in the heat, layers over sunscreen and works on day and night biters. One 3.4 oz bottle covers about a week of daily use for one person.
- Permethrin 0.5% for evening clothes: treat a long-sleeved shirt and trousers at home 48 hours before you fly. It goes on fabric, never on skin.
What travelers report

A guest complained about the number of mosquitoes at this Sigiriya resort. TripAdvisor ↗
- Sigiriya (Jetwing Vil Uyana and Cultural Triangle hotels) · June
Asked whether Vil Uyana supplied mosquito nets and how bad mosquitoes are in June, a destination expert said nets are unnecessary in an air-conditioned room but recommended DEET spray outside it. Another traveler said hotels in the Cultural Triangle generally lacked nets, which were more common at smaller places without air conditioning, and a third said modest guesthouses usually provide nets or air conditioning. Any fever during or within two weeks after your trip means a doctor visit, and you tell them you were in Sri Lanka. So does a skin sore that won't heal in the months after.
Traveler questions
Is dengue bad in Kandy right now?
It has been a hard year. Kandy was among the hardest-hit districts in Sri Lanka's 2026 epidemic, according to press reports of National Dengue Control Unit data. Nationally, cases peaked in July at 29,964 and fell to 4,637 in the first 27 days of September, but in mid-September they were still about 90% above the same week of 2025, the World Health Organization reported. Put repellent on in the morning, again after sweating and before the late afternoon.
Do I need malaria pills for Sigiriya or Polonnaruwa?
The CDC recommends no malaria medicine for Sri Lanka. WHO certified the country malaria-free on September 5, 2016, and in 2026 marked 14 years since the last locally acquired case. WHO also warns that imported cases and the mosquitoes still found across the island keep a risk of reintroduction, which is one more reason to see a doctor for any fever after the trip.
How do I avoid leishmaniasis in the Cultural Triangle?
Cover up from dusk. The sand flies that spread it bite from dusk to dawn, and Polonnaruwa, Anuradhapura and Matale districts have some of Sri Lanka's highest rates, with cases peaking around June to August. Wear long sleeves and trousers for evening safaris, sunset viewpoints and outdoor dinners, ideally treated with permethrin, and put repellent on exposed skin. See a doctor for a skin sore that doesn't heal in the weeks or months after your trip, and mention Sri Lanka.
Do I need the Japanese encephalitis vaccine for Kandy and Sigiriya?
It depends on the trip. CDC says the vaccine is "Not recommended for travelers planning short-term travel to urban areas," which fits a stay in Kandy. It says to consider it for shorter trips that include "visiting rural areas, hiking or camping, or staying in places without air conditioning, screens, or bed nets," and it recommends it for a month or more in areas with the disease. Homestays among the Triangle's paddies come closer to that. Take your itinerary to a travel clinic 4–6 weeks before departure.
