Eswatini's malaria advice covers the east. The Ezulwini Valley sits outside it.

CDC recommends malaria medicine for the eastern areas along the Mozambique and South Africa borders, which by its wording takes in Hlane and Mkhaya. Mbabane, the Ezulwini Valley and Mlilwane in the west fall outside, and transmission is low and seasonal.
Are there mosquitoes in Eswatini? Yes, and in the eastern lowveld they can carry malaria. CDC recommends prescription malaria medicine for the country's eastern areas along the Mozambique and South Africa borders, which by its wording takes in Hlane Royal National Park and Mkhaya. Mbabane, the Ezulwini Valley and Mlilwane in the west fall outside that area, and transmission is low and mostly limited to the November–April rains.
The malaria line, in CDC's words
CDC lists the transmission areas as "Eastern areas bordering Mozambique and South Africa, including the entire region of Lubombo and the eastern half of Hhohho, Manzini, and Shiselweni Regions." It gives no map. Following that wording, Hlane, in Lubombo, and Mkhaya, in the eastern lowveld, are inside, while Mbabane, the Ezulwini Valley and Mlilwane, in the west, are outside. The parasite is mainly Plasmodium falciparum, which is resistant to chloroquine here, and malaria mosquitoes bite from dusk to dawn.
What we suggest you do with this: if your route includes Hlane, Mkhaya or anywhere in the Lubombo lowveld, book a travel-medicine appointment 4–6 weeks before departure and bring the whole trip. CDC lists atovaquone-proguanil, doxycycline, mefloquine and tafenoquine as the options. If you're coming from Kruger, which CDC also lists, one appointment can cover both legs. Our malaria-zone checklist covers the rest.
The numbers are small. A 2021 Malaria Journal study counted 460 cases in 2012, a peak of 1,198 in 2017 and 693 in 2019, with local cases mainly along the Mozambique and South Africa borders. In June 2026 the World Health Organization's Africa office said local transmission has been interrupted in many areas and a significant share of cases are now imported, and Eswatini aims to eliminate malaria by 2028. CDC's advice stays in place regardless, so the clinic visit is still the first step for the east.
When to go
Malaria in Eswatini transmits in the warmer, wetter months from November to April, and from May to October it normally stops, apart from a few riverine hotspots in the lowlands, the 2021 study found. Indoor spraying is scheduled each October ahead of the season. May to October, the dry winter, is the easier time for a lowveld safari, and roughly January to April, the height of the rains, is the hardest. CDC's recommendation for the east doesn't change by month.
Bites, ticks and water
In the west, mosquitoes tend to show up after rain, so close doors and windows at dusk and ask about nets. In the lowveld parks, mosquitoes tend to be heavier in the wet season, and repellent from dusk, long sleeves and a screened or netted room cover the malaria hours. CDC also lists African tick-bite fever, so treat clothing with permethrin and check for ticks after bush walks and rides. It lists schistosomiasis too, a reason to stay out of rivers and dams. CDC's page lists dengue, though no outbreak has been reported. Eswatini asks for yellow fever proof only from travelers 9 months or older arriving from a country with yellow fever risk, including layovers of more than 12 hours there.

Where you're going
Mbabane & the Ezulwini Valley
In the west, outside CDC's listed eastern areas by its wording. Mosquitoes tend to show up after rain, so close balcony doors at dusk and ask about nets.
Mlilwane Wildlife Sanctuary
In the Ezulwini Valley, west of CDC's listed areas by the same wording. Ticks on walks and rides are the thing to check for afterward.
Hlane Royal National Park
In Lubombo, a region CDC lists in full, so the malaria advice applies. Repellent and long sleeves from dusk, and a tick check after bush walks.
Mkhaya Game Reserve
In the eastern lowveld, inside CDC's listed areas whether it counts as Lubombo or eastern Manzini. Cover up from dusk and sleep behind screens or under a net.
The Eswatini kit
- Picaridin 20% travel sprays: odorless, quick to reapply at sundown and small enough for a day pack. One 3.4 oz bottle lasts one person about a week.
- Permethrin 0.5% for safari clothes: treat a long-sleeved shirt, trousers and socks at home 48 hours before you fly. Treated fabric repels and kills mosquitoes on contact through several washes, which suits the dusk-to-dawn hours. Clothing only, never skin.
- A light long-sleeved layer for lowveld evenings.
What travelers report
Paraphrased from TripAdvisor hotel reviews and linked so you can read them in full. Neither is dated, and both come from hotels in the west, outside CDC's malaria areas.
- Royal Swazi Spa, Ezulwini Valley · after rain
A guest said mosquitoes came in numbers after rain through balcony doors that did not seal, and no mosquito nets were provided. TripAdvisor ↗
- Happy Valley Hotel, Lobamba
A guest wrote that mosquitoes are a problem in Swaziland and that, although malaria is said not to be a problem there, getting bitten is still an issue. TripAdvisor ↗
Any fever during or after your trip means a doctor visit, and you tell them you were in Eswatini and whether you went to the lowveld. After time in a malaria area, that rule holds for a year.
Traveler questions
Do I need malaria pills for Eswatini?
It depends on where you go. CDC recommends malaria medicine for the "Eastern areas bordering Mozambique and South Africa, including the entire region of Lubombo and the eastern half of Hhohho, Manzini, and Shiselweni Regions." By that wording, Hlane and Mkhaya fall inside, and Mbabane, the Ezulwini Valley and Mlilwane fall outside. Whether that adds up to a prescription is a call for a travel clinic, made with your route and dates 4–6 weeks before departure. Our malaria-zone checklist covers the rest.
Do I need malaria precautions in Mbabane or the Ezulwini Valley?
By CDC's wording, Mbabane, the Ezulwini Valley and Mlilwane in the west sit outside the areas where it recommends malaria medicine, so everyday bite precautions apply there. A day trip east to Hlane or Mkhaya puts you inside them, so tell the travel clinic about any lowveld day.
What's the best time to visit Eswatini for fewer mosquitoes?
May to October, the dry winter. Malaria transmission normally stops then, apart from a few riverine spots in the lowlands, and returns with the November–April rains. CDC's recommendation for the east applies in every month, so the clinic visit comes first whenever you go.
Can I combine Eswatini with Kruger?
Yes. CDC lists Kruger National Park and Mpumalanga's Ehlanzeni District for malaria medicine too, so take both legs to the same travel-clinic appointment. Our Kruger briefing covers the safari side.
