The CDC recommends prescription malaria medicine for India below 6,500 feet, which takes in Kochi, the Alleppey backwaters and the beaches. Dengue climbed through Kerala's 2026 monsoon and chikungunya hit Thiruvananthapuram in August, and a December–March trip comes after the main peak.

Yes, and the overnight houseboat is the main bite window, because boats on the backwaters moor for the night around dusk. The CDC recommends prescription malaria medicine for India below 6,500 feet, which covers Kochi, the backwaters and the beaches, so the trip starts with a travel-clinic visit.

Dec–Apr tripsDecember to March falls in Kerala's main tourist season, after the southwest monsoon and the dengue peak that comes with it, though counts were still fluctuating in October 2026. April tends to be hot and humid before the next monsoon. The CDC's malaria advice applies in every month.

Kerala and the CDC's malaria line

The CDC places malaria transmission in India "Throughout the country, including the cities of Bombay (Mumbai) and New Delhi (the capital)," with "No malaria transmission in areas >2,000 m (>6,500 ft) elevation." The CDC lists Kerala among the states with such areas. Kochi, the backwater towns and the Thiruvananthapuram coast are low-lying, inside the recommendation.

What CDC says: "CDC recommends that travelers going to certain areas of India take prescription medicine to prevent malaria." For India it lists "Atovaquone-proguanil, doxycycline, mefloquine, tafenoquine." What we suggest you do with this: book a travel-medicine appointment 4–6 weeks before departure and bring every night of the route, Munnar and Thekkady included, plus any rural homestays, plantation bungalows or forest lodges, where the CDC says to consider Japanese encephalitis vaccine. Which medicine, if any, fits you is the clinician's call. Our malaria-zone checklist covers the rest.

Yellow fever vaccine isn't needed when you fly from the US, though India requires proof from travelers 9 months or older arriving from countries it lists as having yellow fever risk.

Dengue and chikungunya in 2026

Kerala's dengue follows the southwest monsoon, and 2026 ran true to form. WHO's South-East Asia office wrote that cases "increased steadily from week 16 but started to decline from week 29," roughly mid-April to mid-July, and in October that they "have continued to fluctuate."

Chikungunya came next. The European Centre for Disease Prevention and Control reported 167 more cases from Kerala in August 2026, "most of them concentrated at Thiruvananthapuram city," and local authorities pointed to the monsoon. The CDC says there is elevated chikungunya risk in India and suggests the vaccine only for stays of about six months or more.

Both viruses travel with Aedes, which bites in daylight with peaks after sunrise and in the late afternoon, so in Fort Kochi's lanes and at beach restaurants repellent goes on over sunscreen in the morning and again before sunset. Our dengue & Zika routine has the schedule. Nipah, back in the news with a case in Kozhikode district in June 2026, spreads from fruit bats and close contact rather than insects; WHO recommends no travel restrictions and advises avoiding bat roosts and fruit with bat bite marks.

The houseboat hour

Overnight boats on the Alleppey and Kumarakom backwaters stop cruising and moor around dusk, sometimes at a bank where guests can't go ashore, just as malaria mosquitoes start their dusk-to-dawn shift. Boats vary, and an air-conditioned bedroom that closes fully tends to be the best defense. Put on repellent and long sleeves before the boat moors and shut doors and windows early, or book a day cruise, which ends before the worst hours.

Cooler nights, and bites tend to be lighter than on the water. Bring these nights to the clinic with the rest of the route, since plantation bungalows and forest lodges are the kind of rural stay where the CDC says to consider Japanese encephalitis vaccine.

The Kerala kit

The spray covers the trip. Add the permethrin for the houseboat night.
  • Picaridin 20% travel sprays: odorless in the humidity, layers over sunscreen and works on day and night biters. One 3.4 oz bottle covers about a week for one person.
  • Permethrin 0.5% for evening clothes: treat a long-sleeved shirt and light trousers at home 48 hours before you fly. It repels and kills mosquitoes on contact and goes on clothing, never on skin. Add a light bed net for homestays without screens.

What travelers report

Paraphrased from two TripAdvisor reviews of Alleppey houseboats and linked so you can read them in full. Neither is dated, and they disagree, which says a lot about how much the individual boat matters.

  • Alleppey (Alappuzha) houseboat, Kerala · evening

A reviewer advised against an overnight houseboat stay: the boat docked early in the evening at a spot where guests could not get off, and mosquitoes and other insects kept them from enjoying the evening. TripAdvisor ↗

  • 21-hour houseboat tour, Alleppey

A reviewer of an overnight houseboat tour through the Alleppey canals reported no mosquito bites and put it down to the boat being spotlessly clean. TripAdvisor ↗

One more rule outlasts the trip. Any fever during your trip or in the year after it means a doctor visit, right away, and you tell them you were in Kerala, India. Dengue usually shows up within two weeks; malaria can take much longer.

Traveler questions

Do I need malaria pills for Kerala?

The CDC recommends prescription malaria prevention medicine for travel throughout India, "including the cities of Bombay (Mumbai) and New Delhi (the capital)," with no transmission in areas above 2,000 m (6,500 ft). Kerala appears only in that high-ground exception, and Kochi, the backwaters and the beaches are low-lying, so the recommendation covers them. CDC lists atovaquone-proguanil, doxycycline, mefloquine and tafenoquine. Which one, if any, fits you is a call for a travel clinic, made 4–6 weeks before departure. Our malaria-zone checklist covers the rest.

Is an overnight houseboat worth it with the mosquitoes?

It can be, if the boat is set up for the evening. Boats moor for the night around dusk, sometimes at a bank where you can't go ashore, so you spend the start of the malaria mosquitoes' shift on still water. Before booking, ask whether the bedroom closes fully and whether the air-conditioning runs all night. Put on repellent and long sleeves before the boat moors and shut doors and windows early. A day cruise ends before the worst hours.

Is there dengue or chikungunya in Kerala right now?

Both rose in 2026. WHO's South-East Asia office saw Kerala's dengue cases climb from mid-April and start to fall in mid-July, and in October described them as still fluctuating. The European Centre for Disease Prevention and Control reported 167 more chikungunya cases from Kerala in August, most of them in Thiruvananthapuram city. Both viruses spread through mosquitoes that bite by day, so repellent goes on in the morning as well as before dusk.

When is the best time to visit Kerala for fewer mosquitoes?

December to March. It falls in the tourist season, after the southwest monsoon of roughly June to September, when dengue peaks; a long-term study of Kerala's cases found them rising in May and peaking in June and July. October and November tend to bring more rain with the northeast monsoon, and April and May are hot and humid. The CDC's malaria advice applies in every month.
