The World Health Organization certified Egypt malaria-free in 2024, and the CDC recommends no malaria medicine. Studies have found dengue and West Nile in Upper Egypt over the years, and on a winter cruise the usual complaint is evening mosquitoes on deck and on riverside terraces.

No malaria, and what Upper Egypt studies show

WHO's certification on October 20, 2024, followed at least three consecutive years without local transmission, and the CDC's Egypt page recommends no malaria medicine. The CDC does list dengue among the bug-borne diseases in Egypt.

A 2012 paper described Aedes aegypti, the dengue mosquito, re-emerging at Egypt's southern border in Aswan and Toshka. An Assiut University team studied a 2016 dengue outbreak in Upper Egypt and found the virus in 60.8% of 51 suspected patients and 44.4% of the mosquito pools it tested, mostly DENV-1. In July 2023 the health ministry confirmed mild cases in Al-Aliqat, a village in Qena between Luxor and the Red Sea. We found no dengue reports from Luxor or Aswan governorates for 2024 to 2026.

West Nile, carried by Culex mosquitoes that bite mostly in the evening and at night, is the other one. A US Navy-led study published in 2010 found West Nile antibodies in 35% of people tested in Upper Egypt, against 14% in Lower Egypt, and isolated the virus from mosquitoes. The CDC's Egypt page doesn't list it.

Yellow fever vaccine isn't recommended or required when you fly from the US. Egypt requires proof from travelers 9 months or older arriving from a risk country, including layovers of more than 12 hours there.

Evenings on deck

On the Nile, mosquitoes are mainly an evening thing, on sun decks, cabin balconies and riverside terraces, and black flies bite by day near the river. December to February brings cool evenings, posters describe spring as the time evening mosquitoes become noticeable, and May to September is the hottest stretch and carries the most risk. Many cruisers see very few. Put on picaridin 20% or DEET 20–30% before dinner on deck and before a sunset felucca ride, wear light long sleeves and trousers after dark, and keep the balcony door shut with the lights off and the air-conditioning running.

Aswan adds sand flies. Researchers found Phlebotomus papatasi resting by day in mud-brick piles under date palms between Bahrif village and the Nile, most of the females blood-fed, and P. sergenti in a Nile village about 4 miles (6 km) north of the city, in studies published in 2001 and 2008. Sand flies bite from dusk to dawn, and the CDC lists leishmaniasis, which they spread, for Egypt, so the evening routine covers island visits and riverside dinners too.

In Luxor, the West Bank's fields and canals and the Corniche hotel gardens are the likelier evening spots, while temple and tomb visits on the desert edge tend to see few biters. For village and garden visits by day, put repellent on over sunscreen, since the dengue mosquito bites in daylight. Our dengue & Zika routine has the schedule.

Sun decks and balconies at dusk draw the complaints, though plenty of cruisers see very few mosquitoes. Keep the cabin door shut and the air-conditioning on in the evening.

Adding Cairo or the Red Sea? The Egypt briefing covers both, including the 2023 and 2024 dengue outbreaks on the coast.

The Nile kit

One spray covers the cruise. Add the backup for a cruise plus a Red Sea week.
  • Picaridin 20% travel sprays: odorless, layers over sunscreen and goes on fast before dinner on deck. One 3.4 oz bottle covers about a week for one person.
  • A full-size backup in the checked bag for a longer trip or a family.
  • Light long sleeves and trousers for evenings on deck and felucca rides.

What travelers report

Paraphrased from TripAdvisor threads about Nile cruises and linked so you can read them in full. Neither is dated, and the season and your cabin change the picture.

  • Nile dahabiya cruise · October and spring

Planning an October dahabiya trip, a traveler asked about insects. One reply said the only mildly annoying insects were mosquitoes in the mid-to-late evening on spring visits, another named black flies as the bigger nuisance, and a third said there are very few mosquitoes most of the year. • Sonesta Moon Goddess Nile cruise

A cruise reviewer said they chose the Moon Goddess partly for a cabin balcony with a sliding glass door they could sit out on, but the balcony was not very useful because of the mosquitoes. TripAdvisor ↗

One rule outlasts the trip. Any fever during or after your trip means a doctor visit, and you tell them you were on the Nile, especially if it comes with a rash, joint pain or headache.

Traveler questions

Do I need malaria pills for a Nile cruise?

No. The CDC recommends no malaria medicine for Egypt, and the World Health Organization certified the country malaria-free on October 20, 2024, after at least three consecutive years without local transmission. That covers Luxor, Aswan and every stop between.

Is there dengue in Luxor or Aswan?

We found no reports of dengue in Luxor or Aswan from 2024 to 2026. Upper Egypt has had it before. An Assiut University team studied a 2016 outbreak in the region, and in July 2023 the health ministry confirmed mild cases in a village in Qena governorate, between Luxor and the Red Sea. The dengue mosquito has re-established around Aswan, so put repellent on over sunscreen for village and garden visits by day.

Are there mosquitoes on a Nile cruise in winter?

Some, mostly in the evening on deck, on cabin balconies and on riverside terraces. December to February tends to be the quieter stretch, posters describe spring as the time they become noticeable, and many cruisers see very few. Repellent before dinner on deck, light long sleeves and a shut balcony door handle most of it.

What bites around Aswan besides mosquitoes?

Black flies bite by day near running water, and some posters name them as the bigger river nuisance. Phlebotomine sand flies bite from dusk to dawn; studies published in 2001 and 2008 found them near the river at Aswan, resting by day under date palms, and the CDC lists leishmaniasis, which they spread, for Egypt. Repellent and long sleeves after dark cover them.
