Langkawi's bites cluster in the mangroves and at the forest edge. Dengue sets the daily routine.
- CDC Level 1 Global dengue (Malaysia listed)
- Travel clinic CDC recommends medicine or vaccines for some areas · which ones
CDC lists malaria medicine for Malaysia's rural, forested areas and doesn't name Langkawi. On our reading, the beach resorts and Kuah fall outside that setting. Dengue is endemic, as across Malaysia, and the bites cluster on Kilim mangrove boats, on Cenang evenings and in villas set among the trees.
Yes, and the bites cluster in a few settings, mainly the mangrove boats in the Kilim geoforest, evenings on the Cenang strip and villas set among the trees. Dengue is the disease to plan for, as everywhere in Malaysia. CDC doesn't name Langkawi for malaria, and on our reading its advice for "rural, forested areas" doesn't reach a beach resort.
Malaria and the forest
CDC reports "Zoonotic transmission of P. knowlesi malaria occurs in rural, forested areas" and lists medicine only for those areas. It doesn't name Langkawi. On our reading, a resort on Cenang, Tengah, Tanjung Rhu or Datai, a stay in Kuah and the day tours fall outside that setting, as our Malaysia briefing also says. The Machinchang and Gunung Raya ranges and the Kilim mangroves are forested, though, so jungle camping, overnight treks or a long stay in the interior are a question for a travel clinic 4–6 weeks before departure.
The same visit can cover Japanese encephalitis vaccine, which CDC recommends for a month or more in areas with the disease and says to consider for shorter trips with rural visits, camping or nights without screens or bed nets. Village homestays or jungle camping fit that wording on our reading. Langkawi had one of Malaysia's four major outbreaks, in 1974, and a 2018 review describes the disease as endemic only in Sarawak. CDC lists Zika for Malaysia, so talk to a healthcare provider if you're pregnant.
Dengue on the island
No 2026 dengue figures for Kedah, Langkawi's state, have reached the Malaysian press. Nationally, the World Health Organization counted 63,752 cases and 60 deaths through late August 2026, above 2025 and the 2021–2025 median. Ministry figures reported in August showed the rise led by Selangor and Kuala Lumpur, while Penang, the nearest state with published figures, ran below 2025 at midyear. The island has had its own rises, including one in late 2022 centered on Kuah and Ulu Melaka, Malaysian press reported. The day-biting dengue mosquito works Kuah, the kampung areas behind the beaches and resort gardens, so put repellent on over sunscreen in the morning and reapply after swims and before sunset.
Mangroves, Cenang evenings and forest villas
Kilim's mangrove boat tours bring midge and sand fly bites on any exposed skin, so treat ankles, wrists, neck and face before you board. Which tiny biters they are hasn't been identified; if they're biting midges, our Barbados sand fly guide explains how they behave. Mosquitoes are scarcer by day and more noticeable in the evening on the Cenang strip and its back streets, so wear light long sleeves and pants to dinner. Villas set among the trees, as on the north coast, get more mosquitoes than open ones. Keep doors and windows shut from dusk and use the plug-in repellers or coils the resort supplies. The macaques at Kilim, the Oriental Village and the waterfalls have a reputation for grabbing food and bags.
When to go
Langkawi's driest months are roughly December to March. The southwest monsoon brings most of the rain from about May to October, wettest around September and October, and dengue tends to follow heavy rain by a few weeks, so September to November is the stretch to be strictest. No month is dengue-free.
Where you're going
Pantai Cenang & Pantai Tengah
The main west-coast beach and shopping strip, where mosquitoes are more noticeable in the evening, on the strip and in its back streets, than by day. Put repellent on before sunset.
Kilim mangrove tours
Boat tours through mangroves and limestone in the northeast, where visitors report itchy midge and sand fly bites. Treat ankles, wrists, neck and face before you board.
Tanjung Rhu & the north coast
Beach resorts backed by forest, where villas among the trees tend to get more mosquitoes than open ones. Keep doors and windows shut from dusk and ask for extra plug-in repellers.
SkyCab & Telaga Tujuh falls
The cable car and the Seven Wells falls in the Machinchang rainforest. Expect bites in the forest and near the falls, so wear repellent and light long pants, and watch your bags around the macaques.
Combining Langkawi with Penang or the capital? Our Penang and Kuala Lumpur briefings cover those stops.
The Langkawi kit
- Picaridin 20% travel sprays: odorless in the humidity, layers over sunscreen and deters midges as well as mosquitoes. One 3.4 oz bottle covers about a week of daily use for one person.
- A full-size backup in the checked bag. Morning, after-swim and sunset coats go through a bottle quickly.
What travelers report
Paraphrased from TripAdvisor threads, a hotel review and a resort Q&A, and linked so you can read them in full. Only the review is dated, from 2011, so read them for where bites happen rather than how busy this year is.
- Four Seasons Resort Langkawi, Tanjung Rhu · 2011
A guest was bitten 23 times in one evening after leaving the room's windows open; extra electric repellers supplied by the front desk seemed to work. TripAdvisor ↗
- Langkawi mangrove tour
A poster said midges and sand flies live in the mangroves and gave them itchy bites that got worse in the heat. TripAdvisor ↗
- Langkawi (Pantai Cenang and the forest) · January (one reply)
Asked whether there are many mosquitoes, one reply said they are scarce by day and more of a risk at night or in forested areas; a January visitor said they were around but not a major problem with repellent in the evenings and a bed net; another poster said to expect bites in jungle areas and near waterfalls and found more mosquitoes on the Cenang shopping strip and its back streets in the evening. TripAdvisor ↗
- Four Seasons Resort Langkawi, Tanjung Rhu
Answering a question about mosquitoes and monkeys, a reply said villas set among the trees get lots of both, while villas in the open see hardly either. TripAdvisor ↗
One rule outlasts the trip. A fever during the trip or within two weeks of getting home means a doctor visit, and you tell them you were in Langkawi, including any forest walks. Until dengue is ruled out, take acetaminophen (paracetamol) rather than ibuprofen, and see bitten in a dengue zone for the warning signs.
Traveler questions
Do I need malaria pills for Langkawi?
CDC doesn't name Langkawi. It reports "Zoonotic transmission of P. knowlesi malaria occurs in rural, forested areas" and lists medicine only for those areas. A beach resort, Kuah and day tours aren't that setting on our reading, the same as our Malaysia briefing. Jungle camping, overnight treks or a long stay in the forested interior of the Machinchang or Gunung Raya ranges are a question for a travel clinic 4–6 weeks before departure. In a study of wild long-tailed macaques, the animal reservoir of knowlesi malaria, all 18 sampled in Kedah in 2020 tested negative, though the paper doesn't say whether any came from Langkawi.
Are there sand flies or midges in Langkawi?
In the mangroves, yes. Kilim's mangrove boat tours are where visitors report itchy midge and sand fly bites, and which tiny biters they are hasn't been identified. They bite any exposed skin, so treat ankles, wrists, neck and face with picaridin or DEET before you board, and leave the bites alone afterward.
Is dengue bad in Langkawi right now?
No recent dengue figures for Kedah state or the island have been reported. Nationally, the World Health Organization counted 63,752 cases through late August 2026, above 2025 and the five-year median, and health ministry figures reported in August showed the rise led by Selangor and Kuala Lumpur, while Penang, the nearest state with published figures, ran below 2025 at midyear. Dengue is endemic on Langkawi, so put repellent on over sunscreen in the morning and again before sunset.
Will my resort villa have mosquitoes?
It depends on where it sits. Villas set among the trees tend to get more mosquitoes, and more monkeys, than villas in the open. Keep doors and windows shut from dusk, run the air-conditioning, use the plug-in repellers or coils the resort supplies, and ask for more if you need them.
