El Nido is inside a CDC malaria area. Here's how to plan the islands around it.

Dengue travels with you to every island, and Bohol declared a province-wide outbreak in late August 2026. The CDC's malaria listing is narrower. It covers the whole province of Palawan, El Nido and Coron included, and the Mindanao Island group, and it leaves out Manila, Boracay and Cebu.

If you read nothing else

Dengue is the everyday risk on every island, spread by mosquitoes that bite in daylight, so repellent goes on in the morning over sunscreen and again in the late afternoon. The CDC recommends malaria prevention medicine for the whole province of Palawan, El Nido and Coron included, and for the Mindanao Island group, which appears to take in Siargao. Manila, Boracay, Cebu and Bohol are outside those areas. If Palawan or Siargao is on your route, see a travel clinic 4–6 weeks before you fly.

Many American trips to the Philippines string several islands together: Manila, then El Nido or Coron on Palawan, Boracay, Cebu or Bohol's Panglao, and for surfers, Siargao. The day-flying Aedes mosquito spreads dengue on all of them, and a morning repellent habit handles most of it. Malaria is where the islands differ. The CDC's listing covers all of Palawan, so the country's most famous beach destination comes with a travel-clinic conversation that Manila, Boracay and Cebu do not.

Dengue comes to every island

Dengue is endemic nationwide, and 2025 started with a surge. Quezon City, part of Metro Manila, declared an outbreak on February 15, 2025, after counting 1,769 cases between January 1 and February 14, nearly three times the 609 of a year earlier.

2026 has been much quieter nationally. The Department of Health counted nearly 118,000 cases from January through August, roughly 40% fewer than in the same months of 2025. Local outbreaks still happen. Bohol declared a province-wide outbreak in late August 2026 after three weeks above the epidemic threshold, with more than 1,800 cases and 21 deaths since early January, according to Philippine news reports. Tagbilaran City, the gateway to Panglao, was among the hotspots.

Chikungunya rides the same mosquito. The CDC says there is elevated chikungunya risk in the Philippines, though the World Health Organization counted 78% fewer cases in 2025 through mid-August than a year earlier.

Aedes mosquitoes breed in small pools of standing water and bite in daylight, with peaks after sunrise and before sunset. Our dengue & Zika daily routine has the full schedule. On an island-hopping trip it comes down to three habits.

Breakfast
Sunscreen, then repellent

Before the boat, the beach or the airport. Sunscreen first; repellent on top.

After a swim or snorkel
Reapply

Island-hopping tours mean hours in and out of the water. Count every long swim as a reset.

Before sunset
Aim low

The second Aedes peak overlaps niknik hour on the beach, and on Palawan and Mindanao the malaria hours begin.

Palawan, Mindanao and the malaria question

The CDC's Philippines page, reviewed in September 2026, recommends malaria prevention medicine for the “Province of Palawan and the Mindanao Island group” and reports no transmission in metropolitan Manila or other urban areas. About 85% of cases are Plasmodium falciparum, the more dangerous species. The CDC notes chloroquine resistance and lists four medicines: atovaquone-proguanil, doxycycline, mefloquine and tafenoquine.

The listing covers the whole province of Palawan, so El Nido, Coron and the countryside around Puerto Princesa all fall inside. Siargao belongs to Surigao del Norte, a Mindanao province, so it appears to fall within the Mindanao Island group, though the CDC does not name it. Manila, Boracay, Cebu and Bohol are outside the listed areas.

What we suggest you do with this: if your route includes anywhere on Palawan, Siargao or other parts of Mindanao, book a travel-medicine appointment 4–6 weeks before departure. Bring the plan island by island, with the nights in each place, and ask whether the CDC's recommendation applies where you will actually sleep. Ask about Siargao by name. Our malaria-zone checklist covers everything around the prescription.

Whatever the clinic decides, malaria mosquitoes bite from dusk to dawn. Repellent at sunset, long sleeves after dark, and an air-conditioned or screened room or a bed net overnight cover most of that exposure.

  • Repellent at dusk as well as by day
  • Screens, air-conditioning or a bed net at night

Japanese encephalitis and the rest of the clinic list

Japanese encephalitis, a mosquito-borne virus, is present in the Philippines. The CDC recommends its vaccine for travelers moving to an affected area, spending a month or more in such areas, or visiting often. It suggests considering it for shorter trips with rural stays, hiking, camping or nights without air-conditioning, screens or bed nets, and does not recommend it for short stays in cities.

The CDC says chikungunya vaccination may be considered for extended stays, for example 6 months or more. There is no CDC Zika notice for the Philippines, though its country page lists Zika, which can pass from a pregnant woman to her baby, so mention any pregnancy plans. Continuing to Bali? It has an active CDC Level 2 Zika notice, covered in our Bali briefing.

Yellow fever vaccine is not recommended and is not required for direct travel from the US. The Philippines does require proof of vaccination from travelers 9 months or older arriving from a country with yellow fever risk, including layovers of more than 12 hours there.

Beach evenings and niknik

On Palawan's beaches the bites that bother visitors most come from niknik, the local name for tiny biting sand flies, or midges. El Nido draws the most complaints, Lio Beach and the island resorts included. Niknik are worst around dawn and dusk, they go for ankles and lower legs, and El Nido reviewers describe them as seasonal.

Spend beach time in the breezier middle of the day, treat ankles and legs before you sit on the sand, and use a lounger or a mat. Beach midges are weak fliers, so an open, breezy stretch tends to be kinder than a sheltered cove. Leave the welts alone, since scratched bites can get infected in the heat, and see a doctor if one turns red, hot or swollen. Our Barbados sand fly guide explains how these insects work.

Boracay has a reputation for evening bites under the palms and for sand flies. Mosquitoes also get into resort rooms without nets or screens, Siargao's guesthouses around General Luna included, so ask for air-conditioning, screens or a net.

Part of Surigao del Norte in Mindanao, so it appears to fall within the CDC's Mindanao Island group malaria listing; ask a travel clinic about it by name. Around General Luna, pick a room with screens, air-conditioning or a net.

When to go

Roughly December to May, the drier half of the year, tends to be easier. Dengue usually builds in the rainy season, roughly June to November, and July to October tends to be the worst stretch. In August 2026 the health department warned of more cases as stagnant water collected. The dry months carry no guarantee, since the 2025 wave peaked in January and February. The CDC's malaria listing names no season, so book the clinic visit whatever month you go.

What travelers report

Paraphrased from TripAdvisor reviews and forum threads and linked so you can read them in full. None is dated, all come from Palawan and Siargao, and individual trips vary with the season, the wind and where you stay.

  • Pangulasian Island resort, El Nido

A reviewer said sand flies ruined their El Nido stay and reported that many guests, including two in their group, ended up hospitalized with severe infections from the bites. This is an outlier, but a reminder not to scratch bites. TripAdvisor ↗

  • Lio Beach, El Nido · a seasonal peak (month not shown)

A reviewer called Lio Beach beautiful but full of sand fleas at that time of year. TripAdvisor ↗

  • Ocean 101 Cloud 9 Surf Resort, Siargao

A guest said mosquitoes were their biggest complaint: the bathroom was full of them at check-in. TripAdvisor ↗

  • Siargao

In a thread about dengue on Siargao, replies recommended an effective repellent, light-colored long clothing and a mosquito net at night, noting that dengue mosquitoes are most active at dawn and dusk but can bite at any time. TripAdvisor ↗

The Philippines kit

The first two cover dengue and beach evenings on any island. Add the third for Palawan and Siargao.
  • Picaridin 20% travel sprays: odorless in the heat, layers over sunscreen and deters both daytime mosquitoes and beach niknik. Carry-on size for the flights between islands; one 3.4 oz bottle covers about a week for one person.
  • A full-size backup in the checked bag for a two-week loop or a family.
  • Permethrin 0.5% for evening clothes: treat a long-sleeved shirt and light trousers at home 48 hours before you fly. Treated fabric repels and kills mosquitoes through several washes, which suits the dusk-to-dawn hours on Palawan and Mindanao. Fabric only, never skin.
  • Anti-itch cream and a light bed net for niknik bites and unscreened guesthouse rooms.

One more rule outlasts the trip. Any fever during or after your trip means a doctor visit, and you tell them you were in the Philippines and which islands you visited. Dengue usually shows up within two weeks; malaria can take longer. Naming Palawan or Mindanao gets you the right test quickly.

Traveler questions

Do I need malaria pills for El Nido or Coron?

The CDC recommends malaria prevention medicine for the whole province of Palawan, which includes El Nido, Coron and Puerto Princesa. Its page does not say whether those towns count as the urban areas it exempts, and the answer depends on where you sleep and for how long. That is a call for a travel clinic, made with your island-by-island plan 4–6 weeks before departure. Whatever is decided, wear repellent from dusk on and sleep in a screened or air-conditioned room or under a net.

Is Siargao inside the CDC malaria area?

It appears to be. The CDC lists the “Mindanao Island group,” and Siargao belongs to Surigao del Norte, a Mindanao province, so read literally the listing covers it. The CDC page does not name Siargao, so ask a travel clinic about it specifically, 4–6 weeks before you fly.

Is there malaria in Manila, Boracay or Cebu?

The CDC reports no malaria transmission in metropolitan Manila or other urban areas, and Boracay, Cebu and Bohol are outside its listed areas. Dengue is the mosquito-borne illness to plan around in those places, so the daytime repellent routine is the main job. If the same trip includes Palawan or Siargao, the clinic visit still applies.

What does Bohol's dengue outbreak mean for a Panglao trip?

Bohol declared a province-wide dengue outbreak in late August 2026, and Tagbilaran City, the gateway to Panglao, was among the hotspots. Dengue spreads through a mosquito that bites in daylight, so wear repellent from breakfast on, top it up after swimming and again in the late afternoon, and choose an air-conditioned or screened room. Check local news before you go, since an outbreak declaration can be extended or lifted.

Do I need the Japanese encephalitis vaccine for the Philippines?

It depends on the trip. CDC recommends it for travelers moving to an area with Japanese encephalitis, spending a month or more in such areas, or traveling there often. It suggests considering it for shorter trips with rural stays, hiking or camping, or nights without air-conditioning, screens or bed nets, and does not recommend it for short stays in cities. Describe your route at a travel clinic and let them advise.

What are the itchy bites on my legs after an El Nido beach day?

Most likely niknik, the local name for the tiny biting sand flies, or midges, on Palawan's beaches. They bite low on the legs around dawn and dusk, and the welts can itch for days. Treat ankles and legs before you sit on the sand, use a lounger or a mat, and leave the bites alone. See a doctor if one turns red, hot or swollen.
